What are the documents that I need to prepare in filing a claim?

In filing a claim, you or your beneficiary(ies) must send us Proof of Claim such as, but not limited to, Death Certificate, Medical Certificate, and Attending Physician’s Statement. We may require you to submit additional documents to help Pru Life UK assess the claim.
